Patients who have already spent many weeks in isolation during liver or kidney transplant, often face multiple trips to the out-patient department to receive treatment and collect medication to prevent organ rejection over many months or years.  Because of the relative rarity of many of these conditions, specialist units tend to be geographically distant from patients, resulting in tiring and time-consuming journeys.

Our experience shows that for patients who undergo intense and prolonged in-patient hospital treatment, the opportunity to receive treatment at home can have a very real impact on their quality of life and rehabilitation following transplant.

Healthcare at Home provides a number of pharmacy based services to support these patients including:-

• The co-ordination and supply of immunosuppressant medication to the referring hospital for inpatients following organ transplantation. On discharge from the hospital, this supply of medication will continue to the patient’s home on a regular delivery schedule, as part of their on-going treatment.
  
  
• Healthcare at Home works closely with the referring hospital in order to provide the patient with up to date doses and to action any dose changes given. We provide a stock check to ensure the supply the patient currently has will be sufficient until the next delivery date. In cases where a dose increase occurs, an additional delivery will be made to last until the next scheduled date.
